Job Description
Umpqua Health is recruiting for a Care Navigator to support patients in accessing timely, effective, and coordinated care. In this role, you’ll serve as the primary liaison between patients, providers, and care teams—helping guide individuals through their healthcare journey from start to finish. Whether it’s scheduling appointments, clarifying treatment plans, or identifying resources to address social determinants of health, you’ll play a central role in reducing barriers and ensuring each patient receives the support they need. Your Impact:
Serve as the main point of contact for patients, families, and care providers, helping to coordinate services, clarify care instructions, and facilitate transitions between levels of care.
Conduct initial assessments to identify patient needs, goals, and potential barriers to accessing services.
Develop and maintain personalized care plans in collaboration with interdisciplinary healthcare teams.
Coordinate appointments, tests, referrals, and follow-up care to ensure timely and seamless access to healthcare services.
Advocate for patients by ensuring they understand their care plans, medication instructions, and available resources.
Provide health education and coaching to patients and caregivers to support self-management of chronic conditions and treatment adherence.
Monitor and document patient progress, updating care plans and making necessary adjustments based on changing needs or circumstances.
Accurately record all patient interactions, assessments, and care interventions in the electronic health record (EHR) system.
Assist patients with navigating insurance processes and connect them to community resources to address non-clinical needs (e.g., housing, food, transportation).
Participate in care team meetings, discharge planning, and quality improvement initiatives to support coordinated, patient-centered care.

Your Credentials:
High school diploma or GED required; associate’s or bachelor’s degree in a health-related field preferred.
Passion for supporting patients through the healthcare system and improving care outcomes.
Excellent verbal and written communication skills, with the ability to build rapport and trust across diverse populations.
Strong organizational and multitasking skills with attention to detail in managing care coordination activities.
Familiarity with healthcare terminology and patient care workflows.
Experience with electronic health record (EHR) systems preferred; ability to learn and adapt to new technology quickly.
Knowledge of insurance coordination, Medicaid/Medicare services, and social determinants of health is a plus.
Ability to work collaboratively within an interdisciplinary team and maintain professionalism in high-stress situations.
Self-starter with strong problem-solving skills and the ability to prioritize in a fast-paced healthcare environment.
Must not be suspended, excluded, or debarred from participation in federal healthcare programs such as Medicare or Medicaid.
