Housing Navigator in our Community Supports program in Temecula, also serving parts of San Bernardino and San Diego. This part-time position helps individuals experiencing or at-risk of homelessness access safe and stable housing by guiding them through housing navigation, benefit advocacy, and service coordination. We are especially seeking candidates with strong bilingual or multilingual proficiency to support our diverse client base.
Opportunity Highlights Community Supports is a Medi-Cal benefit offered under California’s CalAIM initiative, designed to address social determinants of health that impact well-being—such as housing instability. The Housing Navigator plays a key role in delivering Housing Transition Navigation Services, a Community Supports benefit that helps eligible individuals experiencing or at risk of homelessness find and maintain safe, stable housing. This includes conducting individualized housing assessments, developing personalized support plans, securing rental subsidies and documentation, and coordinating move-in logistics. The Housing Navigator also educates clients on tenant rights and provides ongoing advocacy to promote housing retention. This position serves clients throughout the San Diego area and collaborates closely with Enhanced Care Management (ECM) providers and community partners to ensure integrated and person-centered support.

- Hourly pay rate: $23.00 per hour to start, or higher with prior housing navigation or case management experience

This role offers 20-25 hours per week, with shifts typically running from early afternoon to early evening. The position is primarily remote, involving client visits in the field and occasional in-person meetings at our office in San Diego.
Key Responsibilities Under the supervision of the Community Supports Program Manager, you will:
- Conduct housing assessments and develop individualized housing support plans (IHSPs)
- Assist individuals in identifying and securing housing options
- Help complete housing applications and gather documentation (e.g., ID, SS card, rental history)
- Match clients to rental subsidy programs and one-time housing assistance resources
- Advocate with landlords and educate clients on tenant rights and responsibilities
- Support the logistics of moving and coordinate necessary transportation or home modifications
- Coordinate services with ECM providers, community resources, and county homelessness systems
- Submit service authorization requests to Managed Care Plans and document all services in an EHR
- Facilitate transitions to long-term tenancy support services when appropriate
Qualifications - A strong desire to support people in finding and maintaining stable housing
- A compassionate, non-judgmental nature, with the ability to build trust and rapport with vulnerable populations
- Bilingual or multilingual abilities are a plus
- Minimum 2 years of experience working with individuals experiencing homelessness or providing housing-related case management preferred
- Understanding of housing systems, rental assistance programs (e.g., Section 8), and tenant rights
- Familiarity with best practices such as Housing First, Harm Reduction, Motivational Interviewing, and Trauma-Informed Care
- Ability to maintain accurate records and submit required documentation for Medi-Cal services
- Strong interpersonal skills, cultural sensitivity, and the ability to build trust with vulnerable populations
- Ability to work independently and manage a caseload
- Valid driver’s license, auto insurance, and reliable transportation
We are seeking bilingual or multilingual candidates who have at least two years of related work experience in social work, case management, community engagement or customer service. High school graduate or GED required. Prior experience as a community health worker, peer support specialist, medical assistant or similar role is a plus.

Important Note Qualified candidates must complete a criminal background clearance through the California Department of Justice and FBI to ensure eligibility for employment, as well as a pre-employment physical and TB test. This position requires a valid driver’s license, auto insurance and reliable transportation as driving is an essential job function. Housing Navigators are responsible for tasks such as visiting client homes, attending in-person meetings, and engaging with community partners, often in locations not easily accessible by alternative transportation methods. The nature of these responsibilities necessitates the ability to drive to ensure timely, reliable, confidential and effective support for the individuals we serve.
