Key Responsibilities:

• Prepare, dispense, and manage medications tailored to pain management and symptom relief in hospice care.
• Collaborate with interdisciplinary care teams, including physicians, nurses, and social workers, to optimize medication therapy.
• Provide counseling and education to patients, families, and caregivers on medication use and potential side effects.
• Monitor therapy outcomes, adjust treatment plans as necessary, and ensure compliance with hospice care standards.
• Maintain accurate records and comply with regulatory guidelines related to hospice pharmacy practice.

What We’re Looking For:

• Education: Doctor of Pharmacy (PharmD) or Bachelor of Pharmacy (BPharm).
• Licensure: Active and unrestricted pharmacist license.
• Experience: Prior experience in hospice, palliative care, or pain management is preferred but not required.
• Skills: Strong interpersonal and communication skills, with a patient-first approach and an understanding of end-of-life care needs.
